Frenuloplasty is a minor surgical procedure performed to release a short or tight penile frenulum (frenulum breve), which can cause pain, tearing, or restricted foreskin movement during erection or sexual activity. It is typically performed as a day case under local or general anaesthetic, depending on patient preference and clinical setting.

The procedure involves making a small incision in the frenulum to lengthen it, followed by closure with fine absorbable sutures to allow for improved mobility of the foreskin and reduced tension during erections.

Indications include:

  • Short or tight frenulum causing painful erections or tearing

  • Recurrent frenular trauma or bleeding during intercourse

  • Difficulty retracting the foreskin due to frenular restriction

  • Failed conservative treatment (e.g. topical steroid or stretching exercises)

Postoperative care includes:

  • Daily cleaning with warm water and gentle drying

  • Application of antibiotic ointment if prescribed

  • Avoiding sexual activity or masturbation for 2–3 weeks

  • Use of non-restrictive underwear to reduce discomfort

  • Follow-up if healing is delayed or scar tissue forms
